Home and Community Care
Home and Community Care provides support to people living with disability to maintain independence in their own homes.
Activ provides meaningful employment to more than 1,000 people living with disability across metropolitan and regional Western Australia. Employees work in a team environment and are encouraged to develop their skills and abilities in a modern working facility. Through ongoing support from our professional and dedicated staff, employees are encouraged to work towards achieving their personal goals.

Activ’s community and lifestyle supports promote participation in everyday life and provide customers with the opportunity to try something new, Meet new people and develop their independence.
Nothing is quite as good as taking a break to enjoy new and exciting destinations and activities. If you are 18 years and over, have a disability and would like to participate in community activities then we may be able to help you.
The Activ Library is a knowledge centre for disability information in Western Australia. The library has more than 8,000 items including books, DVDs, training materials and journals, with a strong focus on intellectual disability and autism.
Activ Pathways, our registered training organisation, delivers nationally recognised courses and qualifications enabling participants to develop their work skills and increase opportunities for rewarding employment.
Respite services allow carers of people living with disability the opportunity to recharge, relax or attend to other commitments while also providing people with disability the opportunity to meet new people and try new activities.
From the 1st July 2017, instead of the Disability Services Commission providing funding directly to Activ Transport Services, funding will be provided to the person with a disability, based on the reasonable and necessary services and supports outlined in their NDIS Individual Plan. As a result of these changes, Activ will no longer provide a transport service to people living with disability from the 1st July 2017. Click here to find out more about these changes.
